Essentially it monitors and records all the information for every hand that you play in either cash games or tournaments. It helps show whether you are a loose or tight player, loose or tight before and after the flop, how aggressive or passive you are, etc. It is an essential tool for refining your game and being able to discover weak spots that you need to work on.
Lots of good rakeback options at http://www.rakeguard.com/?raf=KRE8R

KRE8R probably has about seventy college funds in his NL5 roll.

Crater nailed it. Just to add to his post Poker-Edge is probably the best tracking software on the market albeit it can be pricey if your playing at the higher limits. On the other hand you are also going to make the monthly fee back in one hand so looking at it from that perspective the price is nominal for a serious cash game player.

Yep, the main uses for tracking software are to monitor your own game to find leaks in your playstyle that could be costing you money. The second main use for tracking software is to monitor your opponents and help you to easily define which players at your table are loose passive, loose agressive, tight agressive etc.

While you should try to pick up the signs of these types of play without software it can be very hard to do so when multitabling, tracking software and a HUD make multitabling infintiely easier for defining what kind of players you are up against.
